歩行練習アシストによる歩行練習を1日40分,週5日,4週間実施する。

Subjects receives gait training using gait exercise assist robot for 40 min per session, 5 days per week for 4 weeks.

1.発症から6か月以上経過している
2.一側性テント上の虚血もしくは出血により片麻痺を呈している
3.平地を歩行補助具(一本杖,四脚杖)や装具の使用の有無に関わらず介助なしで歩行可能
4.トレッドミル上を介助なしで歩行可能
5.研究参加に同意の得られた場合

1.>6 months duration after stroke
2.Hemiparesis of unilateral, supratentorial, ischemic, or hemorrhage stroke
3.Independent in ground ambulation with or without the use of a unilateral assistive device (standard cane, four-pronged cane) and with or without the use of a an ankle foot orthoses.
4.Independent in treadmil gait training
5.Approval for study participation

1.治療者の指示に従えない可能性がある者
2.重度の呼吸循環器疾患,代謝疾患,整形外科的疾患,その他の中枢神経疾患を呈している者
3.強い痙縮を呈している者(下肢Ashworth scale 3以上)
4.ハーネスやロボット装着に制限がある者
5.研究参加の4か月以内に下肢にボトックス治療を受けている者

1.Cannot obey instructions of a healer
2.Significant cardiorespiratory/metabolic disease, or orthopedic injury or other neurological
3.Excessive spasticity (defined as Ashworth scale of greater than 3 at any lower limb joint)
4.Size limitations for the harness/counterweight system or robotic orthosis
5.No botulinum toxin therapy in the lower limbs < 4 months prior to enrollment

GEAR群では、歩行速度が介入前後と退院1ヶ月後に有意に増加したが、トレッドミル群では介入期間中に有意な変化はみられなかった。また、介入前後のTUGと6分間歩行距離の変化量もGEAR群で有意に高かった。GRCは、GEAR群で介入前後と退院1ヶ月後、3ヶ月後に有意に増加した。

Gait speed was significantly increased at after the intervention and 1-month follow-up compared with before the intervention in the GEAR group. Also, mean changes in TUG and 6-min walk were significantly greater in the GEAR group than in the treadmill group when comparing before with after the intervention.The GRC scale scores were significantly higher after the intervention, at a 1-month and 3-month follow-up assessment than that before the intervention in the GEAR group.

During the intervention period, one subject was dropped from the GEAR group after the intervention for personal reasons. Additionally, one subject in the GEAR group experienced uncontrolled arrhythmia between the 1-month and 3-month follow-up visits.
