日本語
DIEP flapによる乳房再建は仰臥位で行うのが主流となっており、欧米の論文では一次一期再建の際,乳腺切除と腹部皮弁挙上を同時進行することで手術時間短縮が可能とされている。しかし欧米と比較して体型の小さい日本人の場合は,十分なワーキングスペースがないため,乳腺切除術終了後に形成外科医が執刀を開始している。よって日本におけるDIEP flapによる乳房再建は手術時間が長くなっていると予想される。
長時間手術は，患者だけでなく，執刀医にとっても負担がかかる。また当院では女性医師が執刀することも多いため,長時間手術は手術を完遂するうえで問題点となってきた。そこで、手術時間を短縮させる目的で,DIEP flapによる乳房再建を砕石位で行う試みを開始した。

Some papers state that flap harvest and breast mastectomy can be accomplished simultaneously in immediate DIEP flap breast reconstruction, to reduce operative time. However, this is not always the case, especially in Asian patients who tend to be shorter than other races. There is not enough working space for breast surgeons and plastic surgeons to perform surgery simultaneously. We attempted to solve this problem by placing the patient in lithotomy position.

2014年10月から2016年8月に同一施設にて同一術者によって行われたDIEP再建の手術時間を、仰臥位群と砕石位群に分けて比較検討する。
