炎症性腸疾患に対する開腹手術を受け、術後に患者自己調節鎮痛法によるフェンタニル静注を用いた患者において、術前炎症関連指標と術後鎮痛薬使用量の関係を検証する。

炎症性腸疾患に対する開腹手術を受け、その術後鎮痛として患者自己調節によるフェンタニル静脈内投与を行なった患者。
術後フェンタニル使用量の継時的な記録がある。
電子カルテ上で周術期の診療記録、看護記録が参照できる。

除外基準/Key exclusion criteria

日本語
肝不全、腎不全、血液疾患の患者。
自分の意思で患者自己調節鎮痛を使用できない場合。
麻薬中毒患者。
オピオイドを常用している患者。
研究に不参加の意思表示がある場合。
記録不備がある場合。

主な結果/Results

日本語
術後24時間のフェンタニル使用量は平均で30.7（標準偏差14.1）mcg/kg/dayであった．術中フェンタニル使用量，現在の喫煙，潰瘍性大腸炎，麻酔中の補助鎮痛薬の使用，手術前1ヶ月以内の生物学的製剤の使用は多変量線形回帰モデルにおいて術後フェンタニル使用量を有意に増加させ，PCA薬液のドロペリドール濃度，年齢，糖尿病は有意に減少させた．一方，有意でない因子としては術前のオピオイド使用があげられた．自由度調整済み決定係数は0.302であった．

Intraoperative fentanyl consumption, current smoking, ulcerative colitis, administration of biologics during the month before surgery, and the use of supplementary analgesics had a significant increasing effect on postoperative fentanyl consumption, whereas droperidol concentration in the PCA solution, age, and diabetes mellitus had a significant decreasing effect. Preoperative use of opioids was a non-significant variable. The adjusted coefficient of determination was 0.302.
