今回我々は、当院においてPCRクランプ法でEGFR陽性肺癌と診断された患者の組織検体を用い、実臨床におけるCobas法におけるEGFR変異の検出感度を再評価するとともに、ダイセクションによるその検出力の変化について検討する。

経気管支鏡下生検で採取した、PCRクランプ法でEGFR変異が検出された検体における、Cobas法でのダイセクション前後のEGFR 変異陽性率を算出し、Cobas法陰性から陽性への陽転化率を算出する。
陽転化率：（ダイセクション後Cobas陽性例/ダイセクション前Cobas陰性例）×100。

・2007年1月～2017年1月の間に経気管支鏡的肺生検で診断されたEGFR変異陽性肺癌患者
・2013年10月以前の検体については改良版のPCRクランプ法でEGFR陽性を確認できたもの

EGFR positive lung cancer samples detected by PCR Clump test from January 2007 to January 2017 at the first biopsy by bronchoscopy.
Improve version of the PCR Clump test was introduced in October 2013 in our hospital. All the samples before the introduction are retested in the improved version and included when positive.

生検検体ブロックをスライスし、腫瘍量など病理学検討を行ったのち、未染色スライドを用いてCobas法でのEGFR変異の検出の（2013年10月以前の検体についてはPCRクランプ法もあわせて）検査を行う。
さらにEGFR変異がPCRクランプ法で陽性でありCobas法が陰性であった検体については、追加でスライスを行い、スライド上でダイセクション後、Cobas法でEGFR変異について再検する。

The block of the subject were sliced and checked for EGFR status with cobas test (the block which were takend before Oct 2013 were re-checked with PCR Clump test).The amonunt of tumour the specimen contains were also pathologically evaluated.
The secpimen with positive EGFR by PCR-Clump test and negative by cobas test were sliced additionally and re-chekced for EGFR status with cobas test after tumor dissection on the slide.
