TAVI成功後のAF患者では、以下のとおりエドキサバンを用いた治療は、NACE事象について、良好に管理されたVKAを用いた治療に対して非劣性であった。
・全死因死亡率、虚血性脳卒中及びICHの発現率は数字的に低かったものの
・主として非致死的である重大な胃腸出血が導く大出血の発現率は高かった。
エドキサバンをVKAと比較して示された点
・虚血性イベント及び全死因死亡率のリスクが低い傾向がみられた

In AF patients after successful TAVI, edoxaban-based therapy was noninferior to well managed VKA-based therapy for NACE events with
- numerically lower rates of all-cause mortality, ischemic stroke, and ICH but
- higher rates for major bleeding that were mainly driven by non-fatal major GI bleeding

合計1426例の患者を、エドキサバン又はVKAに1:1の無作為化計画で登録した。713例をエドキサバン投与群、別の713例をVKA投与群に無作為割付けした。治験薬のエドキサバン投与を1回以上受けた患者は692例であったのに対し、VKA投与を1回以上受けた患者は685例であった。安全性追跡調査を含む研究の全体を完了した患者は、エドキサバン投与が合計526例であったのに対し、VKA投与は475例であった。

A total of 1426 subjects were enrolled in 1:1 randomization schematic to Edoxaban or VKA. 713 subjects were randomized to Edoxaban and another 713 to VKA treatment group. 692 subjects received at least one dose of the study medication edoxaban compared to 685 subjects receiving at least one dose of VKA. A total of 526 subjects on Edoxaban completed the entire study including safety follow-up compared with 475 on VKA.

全体的に、1125例(81.70%)の被験者に投与中に発現した有害事象(TEAE)が1回以上認められた。
TEAEが発現した被験者は、数字的にVKA群の方がエドキサバン群より少なかった(VKA 541例対エドキサバン584例)。しかし、この投与群間差が認められたのは、非重篤なTEAEを認めた被験者数がエドキサバン群で多かったためである(エドキサバン501例対VKA 445例)。重篤又は重度のTEAEが発現した被験者数は群間で同程度であった。薬剤関連TEAEには、同様の傾向がみられた。死亡又は治験薬投与の中断や中止に至るTEAEが発現した被験者数に投与群間差は認められなかった。
最も頻度の高いTEAEは感染症および寄生虫症(459例)であり、次いで心臓障害(396例)、傷害、中毒および処置合併症(314例)、神経系障害(302例)であった。
これらのTEAEの発現率は両投与群で同程度であった。
エドキサバン群の方が発現頻度の高かったTEAEは、血液およびリンパ系障害、呼吸器、胸郭および縦隔障害、胃腸障害、腎および尿路障害、内分泌障害であった。治験を中止した患者は、VKA群の方がエドキサバン群より多かった。また、治験薬の投与期間はエドキサバン群の方が長く、大半の事象(特に出血事象)は、先行して発現したわけではなかった。注目すべき点として、胃腸障害に分類されるTEAEには、上部及び下部消化管出血が含まれ、腎および尿路障害に分類されるTEAEには、血尿が含まれており、いずれも発現した被験者はエドキサバン群の方がVKA群より多かった。なお、DOACのメタ解析の文献において、いずれのDOACにおいても消化管出血が多い傾向であることが報告されている。

Overall, 1125 (81.70%) of subjects had at least one Treatment Emergent Adverse Events (TEAE).
Numerically fewer subjects in the VKA group had a TEAE compared to the Edoxaban group (VKA 541 vs Edoxaban 584). However, this difference between treatment groups was due to a higher number of subjects with non-serious TEAEs in the Edoxaban group (Edoxaban 501 vs VKA 445). The number of subjects with serious or severe TEAE was similar between groups. Similar trends were observed for drug related TEAEs. No difference was observed in the number of subjects experiencing a TEAE leading to death or study medication interruption or permanent discontinuation between either treatment arm.
The most common TEAEs were infections and infestations (459 subjects), followed by cardiac disorders (396 subjects), injury, poisoning and procedural complications ( 314 subjects) and nervous system disorders ( 302 subjects).
Incidences of these TEAEs were similar in both treatment groups.
TEAEs that occurred more frequently in the Edoxaban group included blood and lymphatic system disorders, respiratory, thoracic and mediastinal disorders, GI disorders, renal and urinary disorders and endocrine disorders. More patients from VKA Rx arm discontinued study VKA Rx compared to Edoxaban, longer study exposure in Edoxaban arm and most the events (esp. bleeding event) were not front loaded. Of note, TEAEs classified as GI disorders included upper and lower GI hemorrhage and TEAEs classified as renal and urinary disorders included hematuria, which all affected more subjects within the Edoxaban compared to the VKA group. All DOACs have an tendency towards increased GI bleeding- meta analysis publication.
