要支援・要介護状態のハイリスク高齢者に対し、要支援・要介護状態に移行することを防ぐことを主目的とし、介入の前後比較をすることでその効果を検証する。また事業の実用可能性を評価し、アルゴリズムの適切性を副次目的としたPilot Study

The main objective is to prevent high risk elderly in requiring nursing and long-term care supports, and verify the effect by comparing pre and post the intervention. The pilot study also evaluated the practical feasibility of the project and used the adequacy of the algorithm as a secondary objective.

① 事業支援により結びついたサービスの種類と利用頻度
② 事業支援とは別に既存・新たにつながったサービスの種類と利用頻度
③ 閉じこもり度の評価
④ 高齢者総合的機能評価（CGA）の得点、ロコモ度
⑤ 心理学的指標：QOL（EuroQol-5D-5L）、主観的健康観
⑥身体的指標：アルコール摂取状況（量と頻度）、転倒・骨折の有無、既存疾患の増悪（狭心症発作・心筋梗塞・脳卒中・透析導入等）、他疾患の発症、入院の有無・理由・緊急入院or予定入院・回数、施設入所、死亡、転出、年齢及び性別等
⑦要介護認定の有無・要介護度・サービス利用の内容
⑧入院（理由・回数・期間）、施設入所・転出、死亡

評価時期：開始時、3ヶ月後（平成28年度のみ）、6ヶ月後、12ヶ月後（平成28年度のみ）

Evaluation period: baseline, 3 months(2016), 6 months, 12 months(2016)
