砕石位での手術を予定されている成人で、全身合併症がない、もしくは軽度合併症のみを有する

日常生活でテープかぶれを起こしやすい

主な結果/Results

合計 24 例のデータが得られた。total-Hb、nTHI および TOI の切石位から 60 分間の変化（中央値）はそれぞれ、 +3.09 μmol/L、 +0.083 a.u. および +2.25 % であり、いずれも有意に上昇した（p < 0.05）。回帰分析の結果では、各測定項目の上昇に対して有意な関連を示す因子は認められなかった。

Data were obtained for a total of 24 patients. total-Hb, nTHI and TOI changes at 60 minutes from the lithotomy pisition were +3.09 micromol/L, +0.083 a.u. and +2.25 %, all significantly elevated (p < 0.05). The results of the regression analysis showed no factors significantly associated with the increase in each measure.

参加者背景/Baseline Characteristics

24名のうち、男性18名、女性6名。術前合併症は高血圧14名、糖尿病6名。麻酔法は吸入麻酔23名、TIVA1名。

Of the 24 patients, 18 were men and 6 were women. Preoperative complications were hypertension in 14 and diabetes in 6. Anesthesia methods were inhalation anesthesia in 23 patients and TIVA in 1 patient.

参加者の流れ/Participant flow

2018 年 4 月から 2019 年 12 月に、当院にて麻酔下砕石位で予定手術を受ける成人患者を対象とした。本研究への参加にあたり説明をした後、本人が十分に理解し文書同意が得られた者に対して実施した。

Adult patients undergoing scheduled surgery in the lithotomy position under general anesthesia at our hospital from April 2018 to December 2019 were included in the study. After explaining this study to the patients, the study was conducted on those who fully understood the study and gave their written consent.

測定項目は NIRS（NIRO-200NXTM : 浜松ホトニクス）にて測定される大腿部の総ヘモグロビン濃度（total-Hb）、組織ヘモグロビン指標（nTHI）および組織酸素化指数（TOI）とし、切石位への体位変換直後を基準とした60分後までの測定値を記録した。
