目的1/Narrative objectives1

(1)後期高齢者の健診項目に、高額医療費の使用につながる疾病リスクの特定につながる項目を追加し、早期発見による予防措置（対策）を講じることで入院の回避を可能とすること、(2)健診と対策（(1)）を地域（呉市の一部地域）で実施し、疾病の発症と入院の抑制効果をレセプトデータで比較し、効果を測定することである（探索的研究）。

(1)To make it possible to avoid hospitalization by adding items which can identify disease risks leading to the use of high medical expenses for health check-up items of late elderly people and taking preventive measures (countermeasures) by early detection, (2) To conduct health check-up and countermeasures in the area (a part of Kure City), compare the effect of decreasing the onset of diseases and hospitalization with the receipt data, and measure the effect (A pilot study)

探索的試験であることから、複数の評価項目を設定している。今後、本研究の結果を参考に評価項目の選定を行い、新たな研究につなげる。
(1)疾病（ICD-10：大腿骨骨折、脳梗塞、慢性腎不全、心不全、アルツハイマー病、認知症関連、肺炎関連、慢性下気道疾患）の入院の発生数の変化（年間実人数/疾病、年間入院回数/延べ/疾病、年間入院回数/一人平均/疾病、年間入院日数/延べ/疾病、年間入院日数/一人平均/疾病）
(2)高度救命医療の発生数（年間実人数、延べ人数、一人当たりの発生回数（平均））
(3) 人工透析新規導入者数（年間実人数）

This is an exploratory study; therefore, set various evaluation indicators.
(1) Number of hospitalization (change) (ICD-10: S72, I63, N18, I50, G30, F00-F03, J15 to J18, J69, J40, J41 to J44)(number of hospitalized patients/ICD-10 code, number of hospitalization/ICD-10, hospital length of stay/hospitalization per year)
(2) Number of users/usages of the emergency unit and intensive care unit (number of patients, and the per person)
(3) Number of patients who have newly initiated dialysis

(1)後期高齢者の基本健診に以下の項目を追加実施する。脈拍、呼吸数、SpO2、BNP/Nt-proBNP、総蛋白、アルブミン、ヘマトクリット、ヘモグロビン、クレアチニン、基本チェックリスト、認知機能検査、運動機能検査
(2)健診結果を適切に伝え、保健指導を行い、受診・関係サービスにつなげる

(1) Add new items on the basic health check-up for the late elderly (Pulse, Respiratory, SpO2, BNP/NT-proBNP, Total protein, Albumin, Hematocrit, Hemoglobin, Creatinine, Kihon checklist, Cognitive function test, Timed-Up and Go Test)
(2) Inform the results of (1), provide health education, and refer the elderly to a primary care physician and related services

(1)広島県呉市に在住する後期高齢者で、2018年度と2019年度に実施する基本健診受診者で、本研究の参加同意を得た者
(2)広島県呉市後期高齢者被保険者（2017年度～2021年度に被保険者である者）

(2)The insured of the late-stage medical care system for the elderly (who are eligible from 2017 to 2021 FY)
