高安病（高安動脈炎）は大動脈など比較的太い血管に起こる炎症の結果、血管の狭窄や拡張を生じ多様な症状を示す疾患である。その発見から100年以上経つにもかかわらず、いまだにその原因は明らかにされていない。高安病の成因については、他の多くの疾患と同様に、遺伝的要因と環境要因が複合的に関与していると考えられる。この2つの要因のうち、遺伝的要因として私たちは今まで知られていたHLA-Bの変異に加えてIL12B、MLXなどの2領域における一塩基多型（SNP）が疾患と関連することを全ゲノムワイド関連解析法にて見出した( Terao C, et al. Am J Hum Genet 2013)。しかしながら、これらのうちどの遺伝子が高安病の発症により重要な役割を担っているかについては不明のままである。
また、高安病の診療では、疾患活動性評価に有用なバイオマーカーが乏しいことも問題である。活動性指標として、現在はCRPが広く使用されているが、CRPが陰性でも大動脈の構造的変化が進行する患者も存在する。またIL-6阻害療法中の患者では疾患活動性に関わらずCRPが陰性化するため、疾患活動性の指標としてCRPを使用することができない。これまで我々はマトリックスプロテイナーゼ（MMP）やペントラキシン（PTX）が病勢評価に有用であることを示してきたが（Ishihara et al. JACC 2011.）、構造的変化の予測や、IL-6阻害療法中のバイオマーカーとしての有用性については不十分であり、新たなバイオマーカーの同定が望まれている。本研究では、LRG（Leucine rich α2 glycoprotein）という糖タンパク質に着目し、高安病の新たなバイオマーカーとしての有用性について検討を行う。これまで我々は、LRGが関節リウマチや炎症性腸疾患におけるバイオマーカーとして有用であることを報告してきており（Serada et al. Ann Rheum Dis 2010, Serada et al. Inflamm Bowl Dis 2012）、高安病でも有用であることが期待される。
