若年女性に好発する抗N-methyl-D-aspartate（NMDA）受容体（R）抗体脳炎は、2007年に、Dalmauらにより提唱された疾患であり、NMDA型受容体に自己抗体ができることよる急性型の脳炎で、卵巣奇形腫との関連が指摘されている。本疾患は、発熱・精神症状で初発し、意識障害・痙攣・不随意運動・自律神経症状・呼吸障害を呈し、急性期は重篤で各種治療に抵抗を示し遷延化する致死的な疾患である。卵巣奇形腫を合併する場合には、発症早期に卵巣奇形腫に対する婦人科的手術介入によって回復が早く見込めることも示唆されている。この卵巣奇形腫を伴う抗NMDAR抗体脳炎において、卵巣奇形腫に対して、アプローチとして開腹手術や腹腔鏡手術、術式として嚢胞摘出術や卵巣摘出術が施行されているが、統一された指針はない。今回、若年女性に対する婦人科的手術介入による術後短期における転帰の相違について解析を行い、卵巣奇形腫に対する最適な手術方法を明らかにし、産婦人科としての治療指針を作成するためのデータ収集とする。

Anti-N-methyl-D-aspartate (NMDA)-receptor (R) encephalitis was initially described by Dalmau and colleagues in 2007 when they discovered the anti-NMDAR antibody in a set of young females who had an array of neurological symptoms in association with ovarian teratomas. The typical clinical picture presents with neuropsychiatric symptoms, seizures, dyskinesias, autonomic dysregulation, and dyspnea. The association of ovarian teratoma and anti-NMDAR encephalitis is a serious and potentially fatal disorder that occurs in young women and is thought to be paraneoplastic, in particular due to the rapid clinical improvement after the treatment for the teratoma. However, operative methods (open laparotomy or laparoscopic surgery / cystectomy or ovariectomy) for ovarian teratomas are not uniform in Japan. Objectives are to analyze prevalence and outcome of this association, and increase awareness over this disorder. This study will investigate the timing of operation, surgery method (open laparotomy or laparoscopic surgery / ovarian cystectomy or oophorectomy) for ovarian teratoma in young patients with this encephalitis, and the short-term outcome of the related symptoms after the operation, and will provide practical guidance of the rare disorder for the general gynecologist.
