目的1/Narrative objectives1

日本語
転移性甲状腺癌患者に対する131-I内用療法においては、副作用を軽減して出来る限り安全に施行することが重要であり、本治療に関する重大な合併症である致死的な高K血症を防ぐ事が非常に重要と我々は考えている。そこで、本治療における一連の期間での電解質変化およびRAS系、ADHや副腎皮質ホルモンなどNa、Kの代謝に深く関与する因子を測定することで、どのような症例がどの時期に最も高K血症を発症する危険性が高いのかを解明し、その予防方法を確立するため本研究を行うこととした。

当科にて131-I内用療法の入院治療を行った甲状腺癌症例で、本研究の同意を得られた症例。

除外基準/Key exclusion criteria

日本語
利尿薬、RAS系阻害薬、副腎皮質ホルモンなど、血清電解質の代謝に関与する薬剤を投与されている患者。

甲状腺分化癌にて甲状腺全摘後の患者で、甲状腺ホルモン休薬法によるRAI治療を予定されている者。かつ、甲状腺ホルモン休薬前の血清K値が正常の者。

本研究は、当院に131-I内用療法のために入院した転移性甲状腺癌患者を対象とした前向き観察研究である。入院日（131-I投与1週間前）と131-I投与日の通常の臨床で用いる採血・尿検査（保険診療）に加え、研究用採血を行う。本研究における追加採血は1回あたり約5ml程度であり、2回（入院日と131-I投与日）の採血で約10ml程度である。この量は医学的にみて、患者の健康には殆ど危険性の無い量と考えられる。
尚、観察期間中、血清K値が院内の基準値以上（>5.0mEq/L）となった症例に関しては、採血を1～2日おきに行い注意深く経過観察し、一般に高K血症性不整脈の発症リスクが高いと判断される血清K>5.5mEq/Lとなった際には、血清K値降下療法（K制限食、陽ｲｵﾝ交換樹脂など）を適切に行う。


評価項目：
・入院日と131-I投与日の2回における、FT4、TSH、Na、K、血漿レニン活性、血漿アルドステロン、ACTH、コルチゾール、ADH、血清浸透圧、尿浸透圧。
・患者の臨床背景（年齢、性別、身長、体重、BMI）

解析方法：
・血漿・血清の分離保存は当院９F内科共同実験室にて行う。
・すべて当科の研究費にて外注業者（SRL）にて測定する。
・血清Kの上昇幅と上記の各ホルモンの関連の有無を統計学的に解析する。
