眼表面と腸管マイクロバイオームの恒常性の破綻と造血幹細胞移植後のGVHDによるドライアイにどのような関連があるかを探索的に検討し記述的に要約することを目的とする。

選択基準/Key inclusion criteria

日本語
1)慶應病院眼科ドライアイ外来に通院中または造血幹細胞移植前後に当院入院中の20歳以上の症例
2)書面による同意が得られること
3)造血幹細胞移植後のドライアイ発症時または発症後の症例　
4)造血幹細胞移植後　ドライアイが発症していない症例
5)造血幹細胞移植前受診時にドライアイがない症例
上記1) 2)3)または1)2)4) または1)2)5)を満たす症例


ドライアイの判定基準
1)角結膜所見で蛍光色素試験陽性
またはリサミングリーン試験陽性
または ローズベンガル試験陽性
2)涙液所見でシルマーテスト5mm以下
または 涙液層破壊時間5秒以下
3)ドライアイの症状の存在
1)2)3)がそろっている場合にドライアイの確定診断

1) Adult over 20 years old who is attending dry eye clinic at Keio University Hospital
2) Written informed consent
3) Patients who have newly developed dry eye disease (i.e. chronic GVHD-related dry eye disease) after hematopoietic stem cell transplantation (HSCT).
4) Patients who did not develop dry eye disease after HSCT.
5) Patients who did not have dry eye disease before HSCT.

Eligible for the patients who can be recruited is to fulfill the above number of 1) 2)3)or 1)2)4) or 1)2)5)

Diagnostic criteria for dry eye by Japanese Dry Eye Society 2006

除外基準/Key exclusion criteria

1)書面による同意が得られないこと
2)他の自己免疫疾患の既往がある症例
3)アレルギー性結膜炎、眼表面感染性疾患、コンタクトレンズ装用者
4)対照症例としては移植前にドライアイがある場合

2) No other autoimmune disease
3) No allergic conjunctivitis, ocular surface infectious disease, nor contact lens wearer
4) No dry eye disease before hematopietic stem cell transplantation

１）ドライアイ外来での通常の診療内の眼表面検査であるシルマー検査、角膜知覚、涙液クリアランス、フルオレセイン色素染色、リサミングリーン色素染色、ローズベンガル色素染色、涙液層破壊時間、自覚症状のアンケート検査:Ocular Surface disease Index (OSDI)によるアンケート用紙を用いる。
２）　視力・眼圧・屈折・角膜曲率半径・眼軸長、中間透光体、眼底所見等通常の眼科一
般検査を行う。
３）結膜細胞診（慶應義塾大学医学部　倫理委員会　承認番号20090277の課題で結膜細胞診の手技について承認済みである。一回の検査は5分以内に終了し、通常の診療範囲内で終了する。本検査はドライアイの重症度を調べる検査として30年以上前から行われている。）
４）便検体採取（通常の診療内で行われる検便の方法で行う。）
５）研究者による研究協力者の診療記録の閲覧、年齢、性別、基礎疾患、病歴、眼科全般所見についての閲覧について。
６）以上の臨床所見と結膜細胞診菌叢および腸内細菌叢との対比解析を研究者が行うこと。

1) Shirmer test, Conreal sensitivity, tear clearence, fluorescein staining, Lissamin green staining, tear film break up time, questionnaire for symptom (ocular surface disease index (OSDI))
2) Visual acuity, ocular tension, refraction, length of eye, vitoreoretinal abnormality according to routine examination for eye.
3) Impression cytology and brush cytology for conjunctiva
4) Stool
5) Medical history
6) Researcher will analyse the above mentioned examinations.
