東京都委託を受け東京都医学総合研究所が開発した心理社会的な認知症ケアプログラムの全国的な普及と実装に向けた課題を明らかにする。そのために異なるサービス形態の事業所間でケアプログラムを導入し、実施に係るプロセスの評価を行い、形態間の違いを明らかにする。

We aim to elucidate the barriers to disseminate and implement a psychosocial dementia care programme that has been developed by Tokyo Metropolitan Institute of Medical Science as a delegated project of Tokyo Metropolitan government. We thus plan to introduce the programme into different types of service providers, evaluate the implementation process, and compare the process between the service types.

参加する介護従事者がケアプログラムについての１日研修を受ける。各従事者は２名の認知症の人にケアプログラムを６か月間導入する。１名につき３回の多職種による話し合いを開催し、行動・心理症状の評価、ニーズのアセスメント、ケア計画の立案を行う。

Participating care professionals will complete a 1-day training course of the programme. Each professional will introduce the programme into 2 persons with dementia during 6 months. The professional will hold 3-time multidisciplinary discussion meetings to evaluate level of neuropsychiatric symptoms, identify unmet needs, and establish an action plan for the person with dementia.

認知症の診断を有し、以下の介護保険サービスを利用する者：居宅介護支援、小規模多機能型居宅介護、認知症対応型共同生活介護、介護老人福祉施設

Persons with dementia diagnosis who use following types of long-term care insurance services: in-home care management, in-home multiple service, group home, or special nursing home.

除外基準/Key exclusion criteria

本人または代理人が参加に同意しない、本人の身体または精神機能が急激に悪化した状態にある

The person with dementia and/or his/her surrogate decision maker does not provide informed consent to participate in the study; the person is in an acute deterioration of physical or mental functioning.
