The purpose of this study is to exploratively gather information on whether remote lifestyle intervention (therapeutic guidance through telephonic communication) conducted by Hiroshima University can prevent the exacerbation of diabetes, improve dietary habits, and enhance physical function (exercise habits, step count, etc.) in patients with diabetes who attend their regular physicians in areas lacking diabetes medical centers or core hospitals.

Using a messaging tool, doctors, nurses, and physiotherapists of the research office will carry out diabetes care guidance and patient education. Information related to patient education will be sent three times a week. The Physiotherapists and nurses will conduct remote interventions on lifestyle habits (such as exercise guidance, care guidance, etc.) via phone or video call for the patients.

Type 2 diabetes patients.
Patients who own a personal smartphone and can operate it themselves.
Patients for whom informed consent for participation in this study is obtained in writing and based on their own volition.
Patients with diabetes other than type 2 diabetes (type 1 diabetes, gestational diabetes, diabetes due to other causes).
Patients undergoing dialysis treatment.
Other patients deemed unsuitable for the implementation of this study by the principal investigator and co-investigators.
