To compare survival and recurrence outcomes between patients who underwent a lobectomy + ipsilateral lymph node dissection (LND) and those who underwent a total thyroidectomy + ipsilateral LND for intermediate-risk cN1b PTC with both primary tumors and lymph node metastases in the ipsilateral neck region.

Male and Female
Patients who underwent surgery for intermediate-risk cN1b PTC with both primary tumors and lymph node metastases in the ipsilateral neck region between 2005 and 2012 at Ito Hospital.
Patients with PTC exhibiting high-risk factors, including (i) distant metastases, (ii) any metastatic lymph node 3 cm and over in largest dimension, (iii) T4, or (iv) incomplete tumor resection.
Patients with (v) final pathology indicating non-PTC or (vi) concurrent other thyroid cancers.

Objective: To compare survival and recurrence outcomes between patients who underwent a lobectomy + ipsilateral lymph node dissection (LND) and those who underwent a total thyroidectomy + ipsilateral LND for intermediate-risk cN1b PTC with both primary tumors and lymph node metastases in the ipsilateral neck region.
Design: Retrospective cohort study.
Setting: Ito Hospital, Tokyo, Japan.
Participants: Patients who underwent surgery for PTC between January 2005 and December 2012, excluding high-risk PTCs and concurrent other thyroid cancers.
Main Outcome and Measure: We performed an inverse probability of treatment weighting (IPTW)-adjusted Kaplan-Meier analysis and a Cox proportional hazards regression analysis to compare the patients' overall survival (OS), recurrence-free survival (RFS), and modified RFS (which considered the potential need for a future contralateral lobectomy).
